What is a Home Care Package?

The Home Care Package Program is a government initiative that allows older Australians to receive care at home instead of moving into residential aged care.

Unlike the Commonwealth Home Support Programme (CHSP), which covers short-term or entry-level help, Home Care Packages are designed for people with ongoing or more complex needs.

Funding from a Home Care Package can be used for: 

The Four Levels of Home Care Packages

There are four levels of funding available:

Level 1

Basic needs, such as light cleaning or transport

Level 2

Low-level care needs, such as personal care and home maintenance

Level 3

Intermediate needs, including mobility support and regular personal care

Level 4

High or complex needs, such as skilled nursing and intensive personal care, allied health, dementia or chronic disease support

Each level covers the similar types of services, the difference is the amount of funding and hours available.

The amount of government funding you receive depends on your assessment outcome and your personal financial situation.

The Home Care Package Application Process

Applying can feel complex, but we guide families through every step:

Step 1: Arrange an Assessment

Step 2: Home Visit

An ACAT assessor visits to review your needs.

Step 3: Approval Notification

Government confirms your package level.

Step 4: Income Assessment

Centrelink may calculate your contribution.

Step 5: Package Assigned

You receive a referral code once funding is available.

Step 6: Choose Your Provider

You can select Santa Clara Home Care and sign a care agreement. You can change providers at any time.

Step 7: Start Services

We build your personalised care plan and begin support.

Wait times:

Wait times for levels 2 - 4 may exceed 12 months depending on location, demand and assesed priority. While waiting, you can begin services privately with Santa Clara Home Care and transition once funding is released.

What Does “Approved Provider” Mean?

Only approved organisations can deliver Home Care Packages. As an Approved Provider, Santa Clara Home Care is required to:
